Mandatory Health Insurance Requirement
Students enrolled in the MD, DMD, upper-division Dental Hygiene, Registered Nursing, ARNP, and Audiology & Speech Pathology programs are required to carry major medical insurance. UofL provides a plan through Academic Health Plans (AHP), with claims paid by United Health Care. If you have comparable coverage, you may request a waiver. All plans must be active from the first day of classes.
The following plan types are not accepted for waivers:
- Travel insurance plans (including Travel Guard, Allianz Global Assistance, Seven Corners, Amex Assurance, and Travelex)
- Health-sharing or cost-sharing plans (including Liberty Health Share, Medi-Share, Samaritan Ministries, Christian Healthcare Ministries, CMF CURO, Solidarity, and similar programs)
- Out-of-state Medicaid or other state-based plans — these are limited to emergency hospital use in their home state only
- Short-term medical plans
Waiver Deadlines & Instructions
Waiver Deadlines: August 31 for Fall | February 1 for Spring
Waiver instructions will be sent to your UofL email from Academic Health Plans beginning mid-July for Fall and mid-November for Spring. You cannot submit a waiver before receiving that email. To make changes to your enrollment or waiver status, contact Academic Health Plans at support@ahpcare.com or 1-855-850-4191.
$52.50 Professional Health Fee
All students in the MD, DMD, upper-division Dental Hygiene, Registered Nursing, ARNP, Audiology & Speech Pathology, and Public Health programs are required to pay the $52.50 per semester Professional Health Fee. This fee reduces your out-of-pocket costs for essential health services at Campus Health.
What's Covered
- Unlimited office visits, physicals, and health maintenance visits
- Gynecological exams (outside laboratory charges not included)
- Allergy injections
- Phlebotomy services (lab and x-ray fees billed to patient or insurance)
- In-office lab tests, including strep, flu, pregnancy, and urinalysis
- Annual TB skin testing and flu immunizations
- Post-exposure evaluation, testing, and up to 30 days of prophylactic medications for blood-borne pathogen exposures, including HIV, Hepatitis B, and Hepatitis C
This fee is not health insurance and does not cover hospitalization or services beyond those listed above.
FAQ
Health Science Center students enrolled in Medicine, Dental, Dental Hygiene, Upper Level Nursing, APRN, Speech Pathology and Audiology are required to have health insurance. All students in clinicals are required to carry medical insurance.

After registering for classes, students may enroll in the plan during open enrollment periods and the premium will be automatically placed on your tuition. If you do not have an approved waiver, you will be automatically enrolled in the plan and the major medical fee will not be able to be removed from your account.

Students may qualify to enroll at other times if they have a qualifying event such as birth of a child, death of a spouse, loss of insurance through your spouse\job or reach the age of 26 which eliminates them from a parental policy.

The University-sponsored plan requires students to use Campus Health Services for routine health care. Specialty care is provided through the Anthem BC/BS provider network. For more information visit Louisville.myahpcare.com quicklist.
Referrals are not required but are suggested because Campus Health Services can obtain appointments within the University system which decreases the cost for the student. If you have an after-hour emergency, you may use any immediate/urgent care center or emergency room. These services are subject to annual deductibles and copayments.
Obtaining a referral is not a guarantee of payment but notifies the insurance plan of a pending claim. Final determination of benefits is determined by the insurance company.
Students may use any of the local hospitals in the Louisville area or nationwide. However, UofL Hospital provides deeper discounts for services. Hospital Emergency Rooms are the most expensive form of health care and should be used only for life-threatening emergencies or when all other facilities are closed.
Yes. Using the UnitedHealthcare network will be less expensive if available. See the Louisville.myahpcare.com quicklist for information on how to locate a UnitedHealth Care provider.
Yes. Worldwide coverage and travel assistance is available including air ambulance transfer back to the United States if medically necessary. Please contact Academic Health Plans at (855) 850-4191 for more information.
International students are not covered when they require services in their home country.

- Before You Visit a Provider: Understand the specific procedure to follow when seeking medical treatment under your Student Health Insurance Plan. You are required to use the Campus Health Services Offices as your primary physicians for non-emergency care.
